- Personal Update Form - to request a name change or chosen name, or changes to demographics, citizenship, address, or to declare Indigenous ancestry. Note: Instead of using this form, current students can use WebAdvisor to update their address, phone numbers, and alternate email address. Log in to WebAdvisor, click on your username, select User Profile, and then click on the pencil icon to edit the applicable information.
